- Use estremo for the strongest end of a scale; esterno means outside/external, not extreme.
- Before a noun, estremo often sounds formal or fixed: estremo rimedio, estremo saluto. In everyday speech, molto or davvero can sound more natural.
- For people, estremo can suggest radical views or behavior: posizioni estreme, sport estremi. Don’t overuse it for ordinary emphasis.
